I have just been emailed the updated entry form for the Derby Phoenix meeting at Cadwell in September

With Cadwell our next meeting this time with Derby Phoenix I thought I would get all the forms in one place

Please read the membership form fully and send ALL the information asked on it in an email to dave.smith@dpmcc.co.uk

Dave will reply with your membership number so you can put it on the Cadwell entry form, don't worry about the membership fee, Ben at DPMCC said they will be waiving it

Closing date for entries is the 29th Aug, not sure what happens about late entries, maybe a small extra charge and probably don't make it into the official printed program
