Was the "toothed spacer" the last of the driven clutch plates?? If so, it has to be in place.
That'd cause the springs to tighten down too far and get coil bound and also allow the exposed friction plate to eat the new pressure plate away.
Pull the plate off and fit the toothed spacer, then refit. If the spacer is the driven plate then all should be fine.
